This cover is taken by an employer on behalf of employees and provides payment of a lump sum to the dependents in the event of death of an employee whilst in service. Benefit is determined by multiple of annual salary e.g. *3,*4 uniform figure across the board or per category

1. Critical Illness benefit: A portion of the main cover benefit is payable on first diagnosis of defined critical illnesses during the duration of a cover.
2. Permanent Total Disability benefit: This policy provides for the payment of the benefit if the assured is totally and permanently disabled due to occupational causes
3. Funeral Expense benefit: A pre-selected lump sum becomes payable to the family of the deceased life assured within 24 hours of receipt of notification of death within the cover period.
4. Free cover limit; Members whose sum assured are below the FCL will be covered without undergoing medical examination

2. List of employees ages and salaries
3. Completed proposal form

All employees as deemed eligible by the employer.
